Liability Form
By signing below, I acknowledge that I have read the PLAY Safety Rules and hereby agree to fully comply with such rules because my compliance ensures my own safety and the safety of other guests when participating in the activities powered and provided by PLAY.
I hereby accept any and all responsibility and liability for any injuries or damage that may result from my failure, or my party’s failure, to comply with the PLAY Safety Rules, which shall include, but not be limited to, any and all injuries or damage resulting from activities at PLAY.
- Obey all posted and verbal rules and instructions. PLAY reserves the right to immediately remove a guest from the premises for violating the PLAY Safety Rules or verbal instructions.
- Use common sense to create a safe environment for yourself and other guests. You are responsible for the safety of others and property around them. Always check your surroundings for other guests, computer, TV screens, projector and equipment in and around the bay before swinging a club or bat.
- Only 1 person is allowed in the simulator bay (turf and carpet area), during play. There is a clearly defined sitting / viewing area for all others in the party.
- When swinging the club (practice or real), you must stay within the greenswinging zone.
- No dangerous or disruptive behavior will be tolerated. Using a club, bat or ball for anything other than its intended use for its intended sport or activity is prohibited.
- The frontal projection screen is the only direction that players should be swinging towards. If players are seen swinging and/or aiming the ball in other directions, PLAY reserves the right to stop the game without granting a refund (The same applies to throwing clubs and balls and purposely damaging equipment or property as well).
- No running starts when driving the golf ball (No Happy Gilmore).
- No golf shoes and/or spikes.
- After your turn swinging, please put the club back into the designated bag area or hand it over to the next player. Do not leave clubs lying on the floor (Damage & Trip hazard).
- All parents or legal guardians with children are responsible for making sure that the above rules are enforced. (PLAY is not responsible for monitoring underage children).
- The guest who has their credit card on file will be financially responsible for any damage to PLAY equipment or property caused by anyone in their party that fails to follow the rules or causes damage, other than normal wear of equipment.
- Drinking or eating is not allowed in the turfed hitting area / simulator bay; the seating area is specifically designated for these purposes.
- If any “player” shows signs of being overly intoxicated, a PLAY Team Member may issue a warning and reserves the right to stop the game for the safety of the surrounding customers.
- PLAY is not responsible for any lost or stolen items.
- Guests are required to comply with all local, state & federal laws.
